Program Objectives

By the time the consumer has reached Phase 3, they would have developed:
  • Consistent emotional support/Harm Reduction
  • An opportunity to learn and practice independent living skills (i.e., grooming, money management, shopping, cooking, communication skills, conflict resolution skills, parenting skills, employment skills)
  • Career counseling and guidance
  • Continuing education
  • Job-readiness training and occupational skills development and in most cases, they earn their first paycheck
  • Health care, Medical, and Dental
  • Access to mental health services (therapy, psychiatric treatment, substance abuse treatment)

Eligibility Criteria

Per Housing Program Facility
  • Being homeless, (i.e., Co-Occurring, Mental Health issues, depression, anxiety, bi-polar, etc.) or Chronic Substance Abuse
  • Individuals re-entering from the Department of Juvenile Justice (DJJ), Coordinated Entry System (CES)
  • Chronically Homeless (homeless continuously for one year or 4 episodes of homelessness within 3 years; streets, emergency shelter, emancipated youth, etc.) Placement from Drug Courts, County or State referrals.
  • Referrals for people struggling with poverty and HIV/AIDS and our homeless veterans

The Goal

To work with the participants in a “Three Phase Housing Program Structure” by making available a clean and safe housing facility with the necessary supportive services that will assist them in learning how to become more self-sufficient and confident in choosing and achieving their personal goals to career, work, higher learning, and permanent housing.

My name is Chyna H, I’m 35 and a single stay at home mother to a beautiful 11-year-old girl named Chyane J. Who has autism and reactive airway lung disease. My daughter would often be hospitalized I missed a lot of work. I lost my job, home, and car, we became homeless. Thanks to Community Empowerment and Resources Consultants, CERC and Pathways To Your Future my mind is at ease from there help. They provided safe and clean and peaceful environment for me and my child. The team is amazing, they make sure we have everything we need. They provide food, bedding, cleaning supplies. Covid supplies. Through this program I was able to focus on getting the right resources for my child so I can start working again. I couldn’t ask for a better Transitional housing.

I’m very happy with my home her at Pathways To Your Future and CERC. Before I was in a couple of shelters and God sent me here to E 83rd St Los Angeles. I am back to my old neighborhood, I’m very happy here and I am Not GOING ANYWHERE. I grew up here on 83rd St. My parents bought a house in 1972 here. This is my little success story. I was a homeless Veteran at the time when I moved in this PTYF and CERC’s Rapid Rehousing Program on West 46th St. Los Angeles CA. I strived for excellency, always paid my rent on time and I became house manager. I enjoy living here, when I moved in, I had a job, I met a lot of people that stayed here, we became one big happy family. Since I’ve been here homelessness is not on my mind, I have a peace of mind here, I have my own keys and come and go as I please and the management team they listen to people’s stories. They are very caring and if they have more facilities like this a lot of people will want to stay there, this is the place to go CERC. We need to generate more facilities like this warm, happiness, loving, caring, organization. Home for me I ‘m truly happy. Demitris H.

I have been with the Pathways/ CERC program since May 12th, 2020. I was just getting out of prison and sharing a room with someone else and if you don’t click it can be very very hard, the main thing is it’s all about respect. But by the Grace Of God, the team hear really help me on East 83rd St. I am a 75-year-old man and on my journey, I’ve had many ups and downs. I regular attend group sessions to help me with being around others. I’m able to keep up with my health and finances while with the Pathways/CERC housing program.

I am grateful and happy for the housing program and the housing team. The housing workers are very kind. I have a year and 4 months living in the program and in 2 months more and I will finish the program. I regret not taking the time to do positive things and I feel I wasted a lot of time. My purpose today has changed I’m looking for work and schooling on a daily basis, and to better my English. I desire to be a better person in all areas of my life. Hello I’m Jose N
First of all, I appreciate the program for giving me the opportunity to have a home. I’ve been here with them for a year and a half and I’ve a decent stay and little by little I’ve able to get out of my physical and mental health problems. It’s a little difficult because of coexistence between several individuals and each one with their own problems, but when there is respect and tolerance for others everything will be fine and little by little with my doctors and therapist and help from the program, I know that I would be able to achieve my goals.
